Performs Admission of Client: PHC,PAS,CAS,PCS, Family Care, Transfers, and retro cases.
Maintain communications per telephone call or 2067 with DADS Case Manager or HMO Case Coordinators.
Hire Attendants, process hire packets, orientate attendant on duties, schedule, task, and electronic visit verification.
Perform Attendant evaluations and PCS competency test upon hire and annually.
Assemble and maintain client records, making sure forms are complete and updated and filed completely and securely.
Communicate with MD offices for approved and signed for 3052 for DADS initial services.
Complete and process appropriate forms and paperwork within specified time frames as required by DADS and HMO'S
Enter client data into the computer program (vesta) and keep current data.
Monitor that all approved services are rendered to clients and are properly documented
Maintain attendant work schedules and make changes and assignments as necessary to ensure continuous service to clients.
Complies with PHC/PAS policy manual and any updates.
Maintains documentation on supervisory visits:post hospitalization ; CAS annual re authorizations/HMO authorizations;request increases or changes that client may need or request.
Client annual pas forms; close client charts and 2067's
Document client status on bulletin board.
Report, investigate, document findings and outcomes of complaints(routine and agency self reporting) and notify office manager/or assigned supervisor.
Request for respite hours and get approval from HMO, perform home visit and document.
Make copies of attendant hire packet and new client admission package.
Schedule /plot visits in Vesta program for next visit.
Assist with electronic visit verification
Perform any assigned duties/tasks as assigned by supervisor.
