```Job Summary``` In keeping with our mission to provide patient-centered medical care, the Panel Management Specialist (PMS) will identify patients in need of preventive and chronic disease management services, perform outreach to those patients, and track improvements with the ultimate goal of closing the gaps in care.
Duties and Responsibilities: * Execute the protocol for closing gaps in preventive and chronic disease management services.
* Identify gaps in care by assessing patients' medical records on health insurance registries (from Superior, Driscoll, UHC, etc.
) * Research patients' history in Medisoft and set up appointments for their annual physicals, EPSDTs, and vaccines.
* Prepare and send outreach letters, emails, or text messages to patients.
* Hospital ER Utilization: Contact patient to inform that our clinics are available follow-up and set up follow up appointments as needed.
* Collaborate with clinics, Medical Practitioners (Physicians, Physician Assistants, and Nurse Practitioners) and Clinic supervisors, to close gaps in patient care and, in so doing, maximize incentives the clinics may be eligible to receive from insurance companies and other payers.
* Document outreach and gap closure in medical record and/or registries.
* Prepare reports to track patient interventions in order to demonstrate the value of panel management and outreach activities.
* Maintain a clean work area at all times.
* Perform other duties as assigned.
SKILLS AND QUALIFICATONS: * Required: A high school diploma or equivalent (such as a GED).
* Preferred: Medical Office Specialist, Medical Assistant, or Billing/Coding Diploma.
* Required: Excellent customer relation skills and ability to communicate professionally and effectively with patients.
* Preferred: Working knowledge of medical terminology: CPT, ICD-10, and HCPCS coding and HEDIS quality metric specifications with 6 months of experience.
* Required: Thorough understanding of HIPPA regulations and compliance surrounding access and handling of personal health information.
* Required: The ability to pass a background and drug test.
Job Type: Full-time Pay: $12.
