As a CCM coordinator, you will be working under the supervision of a physician, physician assistant, or registered nurse (RN), scheduling and coordinating telephone check-ins to promote better health.
_*Duties and Responsibilities:*_
* Schedules weekly phone calls with patients; CCMs are responsible for providing twenty minutes monthly of patient education and care coordination support to patients who have multiple chronic diseases, including review of care plan.
* Monitors and reports any changes in patient’s symptoms and behavior
* Establishes a supportive relationship with each patient while providing care coordination support that will ensure access to appropriate services for their health care needs
* Coordinates with the patient’s primary care physician to deliver and coordinate needed services
* Reports immediate concerns and updates to physician or other primary care physician involved in the care of the patient on a regular basis
* Educates patients about health maintenance and disease prevention
* Uses and can document using Electronic Medical Records (EMR)
* Tracks all data of time spent coordinating
* Interacts with other health care professionals as a productive team member
* Forms part of each staff and educational meeting and daily huddles
* Improvises on unforeseen circumstances that arise
* Performs other duties as assigned
_*Qualifications:*_
_Education:_ High School or GED equivalent required
_Licensure/Certification:_ Certified Medical Assistant Required
_Experience:_ At least three years of experience as a medical assistant or other related job experience required.
One year of experience with EMR systems is required.
_Languages:_ Both English and Spanish fluency orally and written required
_Knowledge and abilities:_ The ability to build rapport with patients is strongly preferred.
Ability to communicate with patients from diverse backgrounds.
Strong problem solving and critical thinking skills.
Ability to work independently.
Additionally: a bonus if there is experience in internal medicine and have worked with eClinicalWorks
This is NOT a remote from home job.
