POLICY SETTING RESPONSIBILITIES: Formally responsible for providing input into the policies associated with the job’s purpose and essential responsibilities.
DECISION-MAKING AUTHORITY: Routine decisions include independently solving patient recommendation decisions at the referral level.
SUPERVISORY RESPONSIBILITY: No formal supervisory responsibility but will need to have strong knowledge of and effective in administering hospital policies and procedures.
Makes and implements decisions compatible with organizational goals.
Maintains composure, effectiveness, and flexibility under pressure.
PATIENT CARE PROVIDER RESPONSIBILITY: Direct Responsibility A: Provides screening to those in need.
Time % 20% Relative Importance 5 Task #1: Provides information to patients, families, and other hospital/medical staff concerning all aspects of the patient findings.
Task #2: Provides appropriate recommendation for further services to families as approved by Medical Director.
Task #3: Promotes education about the program to physician groups as well as in the community.
Task #4: Work with Coordinator to create awareness of and brand identity for the High-Risk Follow-up Clinic.
Responsibility B: Coordinates all activity necessary for the program/clinic Time % 20% Relative Importance 5 Task #1: Accepts new referral calls to the program.
Task #2: Coordinates activity for identification, evaluation, and presentation of potential specialty services to Medical Director or attending physician.
Task #3: Assures that all patients testing are completed, assessed and reported.
Task #4: Assists in scheduling of patient appointments, necessary testing, and visits in a timely and efficient manner.
Task #5: Assist with obtaining necessary determination of benefits prior to beginning the evaluation process and charges related to clinic visits.
Task #6: Creates patient summaries to keep multidisciplinary team members apprized of progress through the program.
Responsibility C: Provides follow-up on patients.
Time % 20% Relative Importance 5 Task #1: Provides patients and families consistent and complete follow-up information including signs and symptoms of difficulties, and follow-up appointments on an ongoing basis.
Task#2: Participation of interdisciplinary team conferences to review cases and determine agreed upon plan of care.
Task #3: Ensures that recommendations are carried out appropriately and timely from the patient’s primary care physician.
Task #4: Provides accurate assessment of the patient’s medical condition when communicating with physicians.
Responsibility D: Acts as a liaison between the physicians and other staff.
Time % 20% Relative Importance 4 Task #1: Acts as a patient advocate in maintaining communication and promoting continuity of care between program and referring and community physicians.
Task #2: Establishes and maintains good working relationships with patient, patient support system, peers, physicians and other hospital departments.
Task #3: Responds to telephone inquiries in a timely, professional, and courteous manner.
Responsibility E: Maintains a database of current patients.
Time % 15% Relative Importance 4 Task #1: Maintains an accurate and comprehensive database of all patients.
Task #2: Maintains database to assist with reports, data collection and research.

APRN from the Board of Nurse Examiners for the State of Texas (RN) Current Texas license to practice professional nursing Education and/or Experience: REQUIRED KNOWLEDGE AND SKILLS COMPONENT DESCRIPTION Knowledge Required for completely satisfactory performance in this job is knowledge of nursing functions, basic anatomy and physiology, and diagnostic procedures.
Skills Required for completely satisfactory performance in this job is the ability to show diplomacy and tact to care for families with a child with congenital abnormalities, communicate effectively, and coordinate multiple departments efficiently.
Organization and attention to details are essential as well as the ability to collect and manage data.
The ability to demonstrate and perform generic and unit-based/critical competencies including the specific care of age and diagnosis appropriate patients; demonstrate positive interpersonal skills; utilize time and resources in a cost effective manner; demonstrate flexibility; demonstrate good verbal and written communication skills; delegation; operate basic medical equipment; read and analyze basic medical equipment; excellent keyboarding skills with ability utilize a personal computer to include word processing and basic office equipment.
Formal Education and Experience Advanced Practice Recognition for RN’s with a minimum of three (3) years pediatric patient care experience.
