We are looking for a motivated PHC Case Manager/Field Supervisor to organize and coordinate client care, hire attendants, and provide quality client assistance.
Your role is to create and maintain a pleasant work environment, ensuring high levels of Customer Service, communication and safety.
PHC Case Manager/Field Supervisor duties and responsibilities include coordination of care, assistance with medicaid eligibility, scheduling visits and appointments, hiring attendants, and providing general support to our clients/employees.
Previous experience in the PHC branch would be an advantage but is not required If you have MEDICAID eligibility experience.
A successful Case Manager/Field Supervisor should have experience with a variety of office software (Vesta, email tools, spreadsheets and databases) and be able to accurately handle administrative duties.
Ultimately, Case Manager/Field Supervisor should be able to ensure smooth client coordination care and help to improve company procedures and day-to-day operations.
